301 or 302 — which does it create?

301 (permanent) — the right choice for fixing dead URLs, because it passes link equity to the destination.

Do redirects slow the site down?

Rules are checked early in the request with a single indexed lookup — the overhead is negligible.

Should every 404 be redirected?

No — only URLs with real traffic or backlinks. A 404 for a URL that never existed is fine; the hit counter tells you which ones matter.
